Welcome Former Patients of Dr. Forman!

Most patients will be receiving a copy of our New Patient Package of forms via Medeo or snail mail. For those who are having trouble accessing the forms, we have linked below a copy of our New Patient Package for Dr. Eric Stutz below. The New Patient Package is one PDF file that includes the following:

1) a release form so that we can obtain a summary of your medical record from Dr. Forman,

2) an enrollment form to indicate that Dr. Stutz, will be your new family doctor, 

3) an electronic consent form so that we can communicate with you electronically, and

4) a new patient questionnaire.

Please return the completed package to our office as soon as possible to ensure your spot in Dr. Stutz's practice. You will officially become an active patient in Dr. Stutz's practice once Dr. Forman provides us with your records. 

For security and confidentiality reasons we will not be accepting completed forms via regular email. If you would like to use our secure messaging system to send the forms back to our office please call us at 416-285-3323. The completed New Patient Package can also be faxed back to us at fax number 416-285-1825. Alternatively, it can be mailed or dropped off to:
